Please mail this form, a copy of your driver's license and your check payable to CAPO to: CAPO c/o Ned Costa 9158 Buell Street Downey, CA 90241 -------------------------------------------------------------------------------------------------- IMPORTANT NOTICE: Due to the enhanced security measures at Fort Campbell, it is the responsibility of Robert Page and Ned Costa to provide SFC Yeager, Fort Campbell Public Affairs Office with the following information. You will be required to carry a photo ID (driver’s license for example) at all times while visiting Fort Campbell -- this is a MUST! Again, you cannot get through the gate unless this information is exact and provided to the PAO office not later than 7 days prior to the tour date: June 20, 2007. Please mail Ned Costa a copy of your Driver's License before May 27, 2007. He will package all of them together, prepare a boarding list and forward them to Robert Page to give to the PAO. The only people that we will let on the bus are those who provided us with a copy of your driver's license in time of the deadline.
